Home Care Nurse Visit Private Pay Package

1. Initial Consultation and Assessment

  • Comprehensive Initial Evaluation: $200 (one-time fee)
  • A thorough assessment by a psychiatric nurse to develop a personalized care plan based on the patient's mental health needs.

2. Medication Management

  • Initial Medication Review: $150 (one-time fee)
  • Ongoing Medication Management: $100/visit (45 minutes per visit)
  • Regular medication reviews, monitoring of medication compliance, and adjustment of prescriptions as necessary.

3. Health Monitoring

  • Vital Signs Monitoring: $25/visit
  • Monitoring and recording of vital statistics such as blood pressure, heart rate, blood glucose levels, and weight.
  • Physical Health Assessment: $70/visit
  • Comprehensive assessments to identify any physical health issues that may be impacting mental health.

4. Individual Therapy Sessions

  • Weekly Therapy Sessions: $120/session (1 hour per session)
  • Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and other therapeutic models.

5. Daily Living Support and Health Education

  • Assistance with ADLs (Activities of Daily Living): $40/hour
  • Help with personal hygiene, dressing, grooming, and mobility.
  • Health Education: $60/visit (1 hour per visit)
  • Educating patients and their families on managing psychiatric conditions, medication adherence, and healthy lifestyle choices.
